Harmony: Colonial Life – THERE IS NO UPDATED CENSUS SO IF THE EMPLOYEE ISN'T IN HARMONY, YOU WILL NEED TO ENTER THEIR DATA

  • Accident: Colonial Life

  • Critical Illness: Colonial Life $30k for ee, 50% of ee for spouse and children (max $15k) Plan 2 (Lump Sum CI and Cancer) and plan 4 (cancer only) both have $50 Wellness

  • Hospital Indemnity: Colonial Life Level 2 ($1000 Admission, Daily Hosp conf., Outpatient Surgery and Diagnostic options 1

  • Disability: Colonial Life disability gi: 60% up to $4,000/month – see note below

  • Dental: DO NOT ENROLL-TELL THE EMPLOYEE THEY HAVE AN EMPLOYER PAID DENTAL THAT THEY CAN ENROLL IN VIA THEIR ADP PORTAL.

700 eligible lives-census has been provided to Colonial to load into Harmony

Effective date: July 1

Some Employees are paid bi-weekly and some are semi-monthly but all deductions come out 24 times per year so use 24 pays per year for payroll mode.

OE is In May/June for July 1 plan year.

CIGNA OAP Plan A ($0 in network deductible)

CIGNA OAP Plan C HDHP ($1500 in network deductible)

–PMHCC provides an employer paid STD plan-30 day elimination with 9 week benefit. We can enroll Colonial STD and it will not coordinate with the employer plan.

–PMHCC has an employer provided dental and vision plan. Employees need to enroll using their ADP log in. There is no need for us to enroll Colonial dental/vision.

–PMHCC provides 2 times salary for life insurance and employees can enroll in supplemental term through that plan. – on ADP

The above is available to those employees who are considered full time-30 hours or more per week.
